Forms

Tuition Covenant 2017-2018
This form is returned with your annual registration forms.  It indicates your method of  tuition payment for the upcoming school year and provides the necessary information  for electronic payment of tuition from your checking or savings account.

Lunch Covenant 2017-2018
This form is returned with your annual registration forms.  It indicates your method of  lunch and/or milk payment for the upcoming school year and provides the necessary information  for electronic payment for lunch and/or milk from your checking or savings account.  Click here to see a typical payment plan if you choose monthly, quarterly, or yearly payment plans.  See the hot lunch payment grid.

Child Care & Unscheduled Early Dismissal
This form informs the school office if your child goes to a day care facility after school and also how your child will get home in the event of an unscheduled early dismissal.

Diocese of La Crosse Child Comprehensive Medical Release
This form is completed once per school year, providing information and consent relative to school field trips.  A supplemental form is sent home with your child prior to each specific trip, to be completed prior to the trip.

Free/Reduced Lunch Form
Use this form to apply for free or reduced student lunches through the National Hot Lunch Program.  All information is kept confidential.

Non-prescription medication form
This form is completed when any non-prescription (over-the-counter) medications, including cough drops, are to be taken by your child at school.

Prescription medication form
This form is completed when any prescription medications are to be taken by your child at school.

Parent Statements of Compliance
This form is completed at the beginning of the school year indicating compliance with our Parent & Student Handbook, Internet Use Agreement, and student permissions for school activities.

Registration Form 2017 – 2018
This document contains the forms for registering new students and continuing students each year.  It includes the Registration form and the Authorization for Automatic Payment form, as well as information on tuition policies and payments.

2017-2018 FAIRLetter
Saint Joseph’s Parish has contracted with FAIR (Financial Aid Independent Review) again for the 2017/18 school year.  This will provide us with a tuition assistance needs analysis based on information provided by the applicants.

School Closing Notification System

We are fortunate to be able to include our St Joseph families on the Prescott School District parent notification calling system.

Parent Notification System Letter
Return this form and you will be added to the phone system for notifications.  Once your paperwork is submitted to the school office, you will be added to the phone system.  You will be notified by a phone call for school closings, late starts and other emergencies..  The closing and emergency information will still be advertised on T.V. and radio.

Home & School

The Home & School Association is an organization that belongs to and is made up of all the parents/guardians who have children in St. Joseph Parish School. The association is a service organization whose purpose is to provide ways for parents/guardians to become involved in their child’s school and to support and enhance the work of the school, its staff, and students. Home and School provides parents/guardians with an opportunity to meet and work with other parents/guardians.

The association plans social events throughout the year and leads fundraising activities. Funds raised by the Home and School are used for items or projects to enhance the student environment and education experience. The existence of this organization is based on parents volunteering.
